While highly effective, long-term use of PPIs has been associated with changes in the biological environment that warrant clinical monitoring and consideration.



  • Nutrient Deficiencies: The significant reduction in stomach acidity can impact the body's ability to absorb certain micronutrients. Specifically, long-term PPI use has been linked to potential reduced absorption of Vitamin $\text{B}_{12}$ (requiring stomach acid to be cleaved from protein) and magnesium (hypomagnesemia).

  • Bone Health: There is an observed association between chronic, high-dose PPI use and an increased risk of bone fractures, particularly of the hip, spine, or wrist. This is hypothesized to be related to altered calcium absorption or effects on bone cell function, although the precise mechanism is still being investigated.

  • Infection Risk: The acidic environment of the stomach serves as a natural barrier against ingested pathogens. Chronic acid suppression can increase the risk of certain enteric (intestinal) infections, most notably Clostridioides difficile ($\textit{C. diff}$) infection, and potentially community-acquired pneumonia, by allowing greater bacterial survival.
